Got questions?
Well, we’ve got answers
FORM is for anyone looking to improve their health or performance — whether you’ve never focused on your wellbeing before, are managing a long-term condition, or are an athlete facing a plateau or recurring issues.
FORM is doctor-led, meaning all guidance is grounded in medical knowledge, evidence and clinical experience. FORM takes a holistic approach – blending clinical background, physical training principles and health coaching to help you make sustainable and meaningful change in your life and reach your health goals.
Not at all. FORM is designed to support people at all stages of their health journey, and we have all been a beginner at some point! Whether you’re just starting out or looking to refine an already structured routine, I can help you move forward with clarity and confidence.
All PT sessions are held at my personal studio based in Monkton Farleigh (BA15) near Bath. This is a fully insured space. There is parking available.
Health Coaching takes place online.
Something that you feel comfortable in and can move in along with a water bottle.
Simply reach out via our enquiry form online to book an initial consultation. If you are not sure which package would be best for you, simply contact me and we can go through your options.
Payments are to be made online at the time of booking.
Cancellations made more than 24 hours ahead will incur no charge. No-shows or cancellations made within 24 hours of the consultation will sacrifice that session. Where possible, I will try to accommodate requests to reschedule made less than 24 hours ahead however this is not guaranteed, and if it cannot be provided then the session may be sacrificed.
I have been a GP for 9 years and continue to work in the NHS. Whilst I enjoy my work in the NHS, it lacks the capacity to personalise fitness and wellbeing advice – especially in the presence of disease and for those who are already ‘high functioning’ but still wish to improve. GPs are challenged by time-constraints and can be limited by the services available to them, and most of them are not skilled enough in exercise medicine so they simply cannot specialist expertise or bespoke advice.
I’m afraid not. Whilst I am a qualified and active GP in the NHS, FORM does not provide a GP service. Where a medical issue is highlighted I will communicate this your GP with your consent to ensure that things are not missed.
I will keep electronic records of our consultations and sessions which I will use to summarise. For initial consultations where lots of details are gathered, AI software may be but you will be informed and consented before this. All records are kept securely and safely in accordance to GDPR rules.
Yes when necessary and only with your explicit consent. Usually this is not necessary, however if there is a new problem or one that emerges from our consultations that needs medical attention (diagnostics, imaging or treatment or referral) then I will advise you to seek advice from your own registered NHS GP.
Currently FORM is designed for anyone over 16 years of age but watch this space.
